If the water isn’t clean, isn’t safe to drink or is far away, and if toilet access is unsafe or limited, then we’re not delivering for the world’s children,” said Kelly Ann Naylor, Associate Director of Water, Sanitation and Hygiene, UNICEF. “Children and their families in poor and rural communities are most at risk of being left behind. Governments must invest in their communities if we are going to bridge these economic and geographic divides and deliver this essential human right.”

The report reveals that 1.8 billion people have gained access to basic drinking water services since 2000, but there are vast inequalities in the accessibility, availability and quality of these services. The data shows that 8 in 10 people living in rural areas lacked access to these services and in one in four countries with estimates for different wealth groups, coverage of basic services among the richest was at least twice as high as among the poorest.

Finally, the report highlights new data showing 3 billion people lack basic handwashing facilities with soap and water at home in 2017. It also shows that nearly three quarters of the population of the Least Developed Countries did not have basic handwashing facilities. Poor sanitation and contaminated water are also linked to transmission of diseases such as cholera, dysentery, hepatitis A, and typhoid.
Billions of people around the world are continuing to suffer from poor access to water, sanitation and hygiene, according to a new report by UNICEF and the World Health Organization. On 21st March 2002 multinationals held a private meeting where they created an organisation called "Partners in Africa for Water and Sanitation (PAWS)" specifically to discuss privatisation of water and sanitation in South Africa, Nigeria, and Uganda. The meeting came out with a confidential document that PSI obtained a copy of: this document, prepared by a USA consultancy called PADCO, was solely aimed at persuading South African municipalities that privatisation of water is not a bad idea.

Just one month before this, in February 2002, an internal newsletter of another private consultancy company, RTI, revealed that "RTI and PADCO created the MIIU as a nonprofit company, with its own CEO and Board of Directors, and began laying a foundation for a mature financial market that will facilitate the flow of private capital into municipal infrastructure projects".

The document revealed that PADCO's objectives are to "help MIIU to maintain a pro-private stance in the internal South African debate." USAID has financed PADCO to do much else with infrastructure finance in South Africa. PADCO describes themselves as "providing long- and short-term technical assistance to the MIIU to support the creation of a sustainable framework for private infrastructure investment in the Republic of South Africa."

So which diseases are the biggest threat?

“The most important and probably first type of outbreak that can occur most likely involves the waterborne diarrhoeal diseases. Typhoid fever is one such an example. At present the Western Cape does not have indigenous cholera but that may change, depending on environmental, climatic and human factors,” says Dr Barnes. Let’s take a look at some of these diseases that are bound to hit South Africa if drought and unsanitary conditions persist.

1. Giardiasis

Giardiasis, popularly revered to as “beaver fever”, is a parasitic disease caused by Giardia lamblia. While it’s possible that the disease can live in some people without presenting any symptoms, the symptoms are largely abdominal cramps, diarrhoea, nausea and fatigue. These symptoms typically last for two weeks and can be treated, but like any infection of the digestive system, giardiasis can be dangerous especially in those with impaired immune systems.

2. Typhoid fever

Typhoid fever is caused by the bacteria Salmonella typhi and is transferred between humans. Typhoid can spread quickly and can be fatal if not treated immediately. Typhoid is a risk especially in developing countries and countries experiencing drought. The two most prominent symptoms are high fever and a rash. Other symptoms include abdominal pain and headaches.

3. Hepatitis A

Hepatitis A is an infection of the liver cells. As the body’s immune system tries to fight the virus, the response by the immune system causes liver cell damage and inflammation. According to the World Health Organization, hepatitis A occurs sporadically and often results in epidemics worldwide, and has a tendency to reoccur. Hepatitis A is spread through contaminated food and water.

While hepatitis A doesn’t cause chronic liver failure like hepatitis B and C, it can still cause acute liver failure, which is often fatal. Symptoms can range from mild to severe and can include fever, fatigue, diarrhoea, vomiting and jaundice (yellow skin).

4. Amoebic dysentery

Amoebic dysentery is an infection of the intestines caused by Entamoeba hystolitica. These can attach themselves to the gut lining and spread via the intestinal wall to other organs.

Amoebic dysentery mostly stays inside the intestines and causes bloody diarrhoea, vomiting, nausea and stomach cramps. While the symptoms can be treated and disappear after a couple of days, amoebic dysentery can be fatal if dehydration occurs, especially in those with compromised immune systems. Complications of dysentery can be severe.

5. West Nile Virus

West Nile Virus (WNV) is a mosquito-borne virus that is transmitted to horses and humans through mosquito bites. WNV has occured in South Africa before – the largest outbreak among humans was in the Karoo in 1974, with another outbreak following in the Witwatersrand area in the 1980s.

A study lead by researchers in the USA has found that drought increases the severity of WNV in the USA – a pattern of drought was the dominant weather pattern correlated with the size of a WNV epidemic, researchers say.

Symptoms include feelings of severe illness – a high fever, headache, stiff neck, disorientation, muscle weakness, numbness and paralysis.

Sleeping Sickness Africa

The clinical course of human African trypanosomiasis has two stages. In the first stage, the parasite is found in the peripheral circulation, but it has not yet invaded the central nervous system. Once the parasite crosses the blood-brain barrier and infects the central nervous system, the disease enters the second stage. The subspecies that cause African trypanosomiasis have different rates of disease progression, and the clinical features depend on which form of the parasite (T. b. rhodesiense or T. b. gambiense) is causing the infection. However, infection with either form will eventually lead to coma and death if not treated.

T. b. rhodesiense infection (East African sleeping sickness) progresses rapidly. In some patients, a large sore (a chancre) will develop at the site of the tsetse bite. Most patients develop fever, headache, muscle and joint aches, and enlarged lymph nodes within 1-2 weeks of the infective bite. Some people develop a rash. After a few weeks of infection, the parasite invades the central nervous system and eventually causes mental deterioration and other neurologic problems. Death ensues usually within months.

T. b. gambiense infection (West African sleeping sickness) progresses more slowly. At first, there may be only mild symptoms. Infected persons may have intermittent fevers, headaches, muscle and joint aches, and malaise. Itching of the skin, swollen lymph nodes, and weight loss can occur. Usually, after 1-2 years, there is evidence of central nervous system involvement, with personality changes, daytime sleepiness with nighttime sleep disturbance, and progressive confusion. Other neurologic signs, such as partial paralysis or problems with balance or walking may occur, as well as hormonal imbalances. The course of untreated infection rarely lasts longer than 6-7 years and more often kills in about 3 years.

The WHO African Region continues to bear the brunt of the global burden of malaria. In 2015, 88% of global cases and 90% of global deaths occurred in the African Region. Contamination Zone - Nicotine Spill.

First Responders should use a NIOSH-certified Chemical, Biological, Radiological, Nuclear (CBRN) Self Contained Breathing Apparatus (SCBA) with a Level A protective suit when entering an area with an unknown contaminant or when entering an area where the concentration of the contaminant is unknown. Level A protection should be used until monitoring results confirm the contaminant and the concentration of the contaminant.

LEVEL A: (RED ZONE): Select when the greatest level of skin, respiratory, and eye protection is required. This is the maximum protection for workers in danger of exposure to unknown chemical hazards or levels above the IDLH or greater than the AEGL-2.

A Totally-Encapsulating Chemical Protective (TECP) suit that provides protection against CBRN agents.

Chemical-resistant boots with a steel toe and shank.
Coveralls, long underwear, and a hard hat worn under the TECP suit are optional items.

Nicotine is a naturally occurring toxic chemical found in tobacco plants. It has a fishy odor when warm. Cigarettes, cigars, other tobacco products, and tobacco smoke contain nicotine. Worker exposure may occur during processing and extraction of tobacco. At one time, nicotine was used in the United States as an insecticide and fumigant; however, it is no longer produced or used in this country for this purpose. Nicotine affects the nervous system and the heart. Exposure to relatively small amounts can rapidly be fatal.

The term immediately dangerous to life or health (IDLH) is defined by the US National Institute for Occupational Safety and Health (NIOSH) as exposure to airborne contaminants that is "likely to cause death or immediate or delayed permanent adverse health effects or prevent escape from such an environment."

Assigned Protection Factor (APF) means the workplace level of respiratory protection required for a dangerous atmosphere.

Emergency or planned entry into unknown concentrations atmosphere with Nicotine present or IDLH conditions:

(APF = 10,000) Any self-contained breathing apparatus that has a full facepiece and is operated in a pressure-demand or other positive-pressure mode
(APF = 10,000) Any supplied-air respirator that has a full facepiece and is operated in a pressure-demand or other positive-pressure mode in combination with an auxiliary self-contained positive-pressure breathing apparatus

Agent Characteristics

APPEARANCE: Crystals or crystalline powder.

DESCRIPTION: Fentanyl is a member of the class of drugs known as fentanyls, rapid-acting opioid (synthetic opiate) drugs that alleviate pain without causing loss of consciousness (analgesic). Fentanyl depresses central nervous system (CNS) and respiratory function. Exposure to fentanyl may be fatal. Fentanyl is estimated to be 80 times as potent as morphine and hundreds of times more potent than heroin. It is a drug of abuse. Fentanyl (and other opioids) could possibly be used as an incapacitating agent to impair a person’s ability to function. In October 2002, the Russian military reportedly used “a fentanyl derivative” against terrorists holding hostages in a Moscow theater; 127 of the hostages died. (It is unclear whether the gas used also included other chemical agent(s).) Fentanyl is odorless.




METHODS OF DISSEMINATION:

Indoor Air: Fentanyl can be released into indoor air as fine particles or liquid spray (aerosol).
Water: Fentanyl can be used to contaminate water.
Food: Fentanyl can be used to contaminate food.
Outdoor Air: Fentanyl can be released into outdoor air as fine particles or liquid spray (aerosol).
Agricultural: If fentanyl is released into the air as fine particles or liquid spray (aerosol), it has the potential to contaminate agricultural products.

ROUTES OF EXPOSURE: Fentanyl can be absorbed into the body via inhalation, oral exposure or ingestion, or skin contact. It is not known whether fentanyl can be absorbed systemically through the eye. Fentanyl can be administered intravenously (IV), intramuscularly (IM), or as a skin patch (transdermally).
